Insurance & Fees
My practice is a direct pay practice. This means that I collect my full fee directly from you at the time of service. There are benefits to this, including greater control over treatment frequency and duration, no requirement for a diagnostic code, no hidden fees, and greater control over confidentiality. There is of course, a greater cost to you. However, research suggests that individuals who pay for their therapy have better outcomes likely because they want to "make the most" out of their sessions.
Often, insurance companies will reimburse you for a portion of the cost of my services. To do this, you can submit your expenses to your insurer for reimbursement naming me as an out-of-network provider. I am happy to provide you with invoices and/or appointment summaries with diagnostic codes to aid that process. Additionally, if you give me permission, I will file with your insurance company to reimburse you directly. This way, you do not have to try to manage your insurance company without support.
If you wish to submit for out-of-network benefits on your own, I encourage you to contact your insurance company to find out what out-of-network benefits are offered as out-of-network benefits vary widely between insurance companies and plans. Each company also has their own medical claim form. To further assist you in this process, I have linked these forms for several insurance companies here:
Anthem Anthem Healthkeepers Cigna Humana
Optima Health United Behavioral Health
My rates can be found here. I accept cash, check, or credit card (including health savings account cards).
Yearly Rate Increase Notice: Please note that once a year, around January 1st, I typically raise rates by $5.
Good Faith Estimates: As of January 1, 2022, I am required to provide a Good Faith Estimate to any clients who are not using any insurance benefits (meaning you are not using any out of network benefits) or who are uninsured. Please find a copy of my Good Faith Estimate form here. This is something I will fill out and provide to you after I understand what services you are requesting. We will review your personalized Good Faith Estimate during our first session, or before if requested. Additionally, you may find this Notice of Protections against surprise billing information helpful.
Cancellation Policy: I request 24 hours notice when you wish to cancel or rearrange an appointment. There is no fee for changing and cancelling a session when you give me more than 24 hours notice. Cancellations within 24 hours of the appointment will be charged a late cancellation fee equal to the session fee. Of course I understand that there are sometimes emergencies which mean you cannot let me know 24 hours ahead of time (e.g., illness), however to the best of your ability, please give me time to offer that appointment to others. If you have not given me notice and you do not show up for a scheduled appointment, I will charge you the cost of the full session fee. After 3 emergency cancellations, a $80 charge will be incurred for all subsequent emergency cancellations within a calendar year. There are often many people waiting for an appointment, and I reserve each time slot specifically for you when you schedule. I am always happy to rearrange our appointment if needed, with adequate advance planning.
